What You’ll Do:

As a Crisis Case Manager, you’ll be on the front lines of mental health care, responding to individuals in urgent need—right where they are, whether that’s in their home, at work, or in the community. Every day is different, and your impact will be immediate and meaningful.

In this role, you will:

· Deliver mobile crisis intervention services in real-time, meeting individuals face-to-face in their moment of need.

· Support patients within our Crisis Intervention Center by helping them reach treatment goals and avoid hospitalization when possible.

· Coordinate with treatment teams to monitor patient progress and ensure continuity of care.

· Provide crisis support via telehealth when in-person response is not feasible.

· Respond to critical incidents with flexibility, including after-hours and on-call shifts.

· Accurately complete clinical documentation in compliance with regulatory and organizational standards.

· Undergo extensive training in trauma-informed care, crisis de-escalation, suicide risk assessment, emergency protocols, and more—to ensure you’re well-prepared and well-supported in the field.

· Provide transportation to clients when needed, making mobility a key part of your service.

· Collaborate with other programs (such as Screening & Referral or Assertive Community Treatment) to deliver wraparound support to clients.

This is more than a job—it’s a mission. You’ll be helping individuals regain stability and dignity during some of the most challenging moments of their lives.
